Post Title: Social Prescribing Link Worker

Salary  £28,000 pro rata Contracted hours 26.5           

The Social Prescribing Link Worker will receive referrals from Clover GP practices within the Seven Hills Primary Care Network, providing practice (bookable clinics) and community-based consultations with patients.

The Social Prescribing Link Worker will deliver a range of activities that includes one-to-one work and leading on group activities that improve the health and wellbeing of people living in the area served.

They will work closely with health professionals, service providers, local community, voluntary and faith organisations to enable clients to achieve improved and sustainable physical and mental health and wellbeing.

Social Prescribing Link Worker will offer structured support for clients to change behaviour(s) and increase capacity and capability to make changes. This will be achieved by building the clients motivation, confidence for change, delivering self-care strategies, connecting with community assets and through setting and supporting clients to achieve goals including; increasing social connections, financial stability, movement closer to/retention in work, improving relationships, support clients to quit smoking, reduction in alcohol consumption, weight management, increasing in physical activity etc.

SPLW will work with other local service providers to maximise the potential health and wellbeing outcomes for clients

SPLW will be passionate about improving the health and lives of their clients and will be able to use their personal qualities, skills and experience to motivate, inspire and support clients to achieve goals based on what the clients want to achieve and what improvements they would look like to make.

Post Title: Children, Young People and Families Social Prescribing Link Worker

Salary: £25,000 pro rata Contracted hours: 28  

The Children, Young People & Families Social Prescribing Link Worker will receive referrals from the Clover GP practices within the Seven Hills Primary Care Network, providing practice (bookable clinics) and community-based consultations with patients.

They will deliver a range of activities that includes one-to-one work and leading on group activities that improve the health and wellbeing of young people and their families, living in the area served.

They will work closely with health professionals, service providers, local community, voluntary and faith organisations to enable patients to achieve improved and sustainable physical and mental health and wellbeing.

This Link Worker will offer structured support for patients to change behaviour(s) and increase capacity and capability to make changes.

This will be achieved by building the patients’ motivation, confidence for change, delivering self-care strategies, connecting with community assets and through setting and supporting patients to achieve goals including; increasing social connections, increasing opportunities, financial stability, movement closer to/retention in work, improving relationships, support patients to quit smoking, reduction in alcohol consumption, weight management, increasing physical activity etc.

Background Information

The funding for these roles has come through the PCN Test Site Programme; please see details below. These roles will be working with the Clover Practices within the SevenHills Primary Care Network (PCN).

• The PCN Test Site Programme is a partnership between seven Integrated Care Boards, NHS England (NHSE) and 22 PCNs. These PCNs represent a range of population: urban, rural, coastal, deprived, and affluent. 

• The programme has been set up as a direct response to the challenges and pressures that PCNs and their practices are facing.  Better understanding of the challenges will lead to better local and national policy. 

• The main aim of the Test-Site Programme is to assess the gap between demand and capacity in general practice, and to see if well-functioning PCNs can close the gap.

• The programme kicked off on 18th September 2024 and will run until March 2027.

• There is a range of areas the PCNs want to work on to help close the gap and to deliver better care for their patients.
