Accessing our services
Through a referral to Darnall Well Being, a client can be encouraged to participate in healthy activities and increase their social connections.
For patients we can offer a non-medical, community based signposting and support service.
Our Health Trainers are trained in diabetes and chronic pain management, and offer 6-8 weeks of support that is individually tailored to the client. The Health Trainer and client set goals together collaboratively, an approach which has been shown to increase motivation and lasting positive behaviour change.
Some of our groups need to be booked in advance, so please contact us if you are interested in coming along to one of our groups. We can then make sure there is a space for you and there will be someone to welcome you on your first visit, to help you settle in.
How to make a referral
As a GP or health professional you can use whichever of the following methods suits you best:
- If you are a clinician at the following Practices Darnall Primary Care Centre and Highgate Surgery, you can make a referral via System 1, using the GUided Referral Unit (GURU) task.
- Use our Social Prescribing Referral Form
- Alternatively call the office between 9-4pm Monday to Thursday, 9-3pm Friday.
- Post Tray at York Road Surgery. (You will receive feedback when the client has been contacted and engaged.)
Support someone to access Darnall Well Being
Clients can also self-refer to access our services. If you know of someone who could benefit from any of our services, they can get in touch via phone or email and we can help you to get the support you need from our groups or Health Trainers.
